    Very good condition with slight edge wear.
    It looks like it was originally pink but has faded from light exposure, especially on the front and back. Marked Form 452 and “Additional Copies may be obtained from Horlick’s Malted Milk Corporation Racine, Wisconsin.”
    This is a rare piece of ephemera by Herman Bundesen best known for his book on Baby care published in many editions in the 1930s and 40s.
    He served as Chicago Health Commissioner, Cook County Coroner, and the President of the Chicago Board of Health.
    He was a Public Health activist with special focus on infant and child care and health.
